Speak to Almira on Lady Vengeance to initiate Invaders, The Sallow Man, and The Key to Freedom, as well to acquire any gear. Check in with Tarquin for gear as well, then take the small boat and head off to the beach.

Climb the vines at the Temple of Amadia to initiate Up in the Clouds.
Nameless Isle - North
Find the Black Ring Alchemist and his compatriots south of the Temple of Rhalic; buy goods and skillbooks from them if you like, then approach the Lizard Dreamers. Convince them you're not their concern or fight them; if you fight them, pick up the Black Mirror afterward and use it to progress Invaders and The Sallow Man.
Make your way through Lava Fields toward Shadow Prince and let The Red Prince speak with him to progress The Red Prince. Approach the Altar of Zorl-Stissa nearby and accept its challenge to learn she is aligned with the sun.
Talk to The Watcher near the Lunar Gate to initiate The Watcher's Mercy.
Travel to Elven Temple where you'll find Shriekers; use Purging Wands you've found throughout the map to eliminate two of them, after which Knight of Tir-Cendelius will appear and tell you Bishop Alexandar is nearby.
On your way to Bishop Alexandar, the Elven High Priestess will approach you and ask that you seek out the Elven Scion, initiating The Mother Tree.
Head to East Cave and speak to Krug the Troll to advance The Sallow Man. Once you're done with the Sallow Man, touch the black mirrors to advanced The Red Prince.
Go to War Room to find Haft of the Swornbreaker and progress The Key to Freedom.
Nameless Isle - South
Near East Cave you'll find two wolves fighting over a Large Ruby. Distract them with meat then take it and use it to initiate Running Like Clockwork, which you must complete to move the main quest forward.
Enter Murky Cave to acquire Eternal Stormblade. Take it from the stone then fight the sentinels that come to life. Loot them afterward to acquire Phase Capacitors and an eternal artefact.
Approach the Orcish Temple to initiate The Drowned Temple, then find the conjurer in Black Ring Camp who is flooding the temple using portal and kill her to drain the area. Enter the temple and find the altar at the end of the long hallway (be mindful of traps). Pray at it and let Vrogir drain you of vigour to learn he is aligned with the sun. Find two eternal artefacts nearby.
With all alignments now learned, head to Lunar Gate and align the pillars as follows (noting the sun symbol looks like a star and the moon symbol an arrowhead): Human (Sun), Elf (Moon), Dwarf (Moon), Orc (Sun), Imp (Moon), Wizard (Moon), Lizard (Sun). Next, use a lightning skill or scroll on the eternal conductor, then hit the lever to unlock the gate.
Speak to your companions and decide which of you should ascend. Note if you decline Lohse, you'll have to fight her, the Red Prince doesn't particularly want to ascend, and Ifan very much wants to ascend. Also, the non-skill check options are best if you are low on persuasion.
